OTC Authorization
To keep students safe and comfortable, New York State requires that all over-the-counter (OTC) products used in the health office — such as Tylenol, Vaseline, or antiseptic wipes — have a written order from your child’s healthcare provider, along with your consent.
This allows us to help with common issues such as minor headaches, chapped lips, or itchy bug bites during the school day. To make it simple, please have your child’s provider complete the Healthcare Provider Authorization Form - OTC Medications. Many offices can do this without an appointment if your child has had a recent visit. The completed form can be faxed or emailed directly to your child’s school health office.
We are happy to provide these supports for your child and appreciate your help in keeping our students healthy, safe, and ready for their day.
